The Sona Access Program is a service that provides personalized assistance for managing difficult medication regimens.
The goal of the program is to help patients take the correct medications at the right time to improve their health. Although it sounds simple, the value that the Sona Access program provides to caregivers is immeasurable.
Below are stories of how the Sona Access Program has helped ease the load of caregivers:

Mr. Smith*
In May of 2017, Mr. Smith’s daughter came to us in desperate need of help.
Her Dad had been in living an assisted living facility for over a year, simply because he did not have the ability to manage his medications by himself.
He wanted so badly to be able to go home and take control back of his life, but Mr. Smith’s daughter worked full-time and did not have the time to manage his complex medications.
We told Mr. Smith’s daughter about the Sona Access program and the ability to utilize a different packaging option that would help her father. We established Mr. Smith in the program, synchronized his medications, packaged them in 7 day pill packs, and began hand-delivering them to him for free.
6 months later, we are very delighted to share that Mr. Smith has moved back to his home and is living independently! He communicates directly with our Sona Access team during his care call, and he no long worries about his medications.
Mr. Smith’s daughter has been so grateful, and we could not be more proud of the progress he has made!
Ms. Jackson*
Ms. Jackson was referred to the Sona Access Program from a skilled nursing facility where she had been admitted for over a year. During this time, Ms. Jackson was diagnosed with diabetes.
Unfortunately, during her lengthy admission her spouse became very ill and passed away. This left Ms. Jackson returning home alone.
The new diabetes diagnosis brought great concern for her and her family – especially with how to test her blood glucose levels.
Together we came up with a plan!
The Sona Access team delivered a glucometer and all of the needed testing supplies to Ms. Jackson while she was still in a facility. She was able to use her own supplies and be confident in how to use them once she returned home.
Ms. Jackson also made a decision to have her medication packaged for her and delivered monthly, easing the strain of depending on family members picking up medications and filling her pill box.
This family was so thankful for the extra support the Access Program was able to provide. They told our team that this was a relief off of their shoulders and they could finally start the healing process.
Ms. Clark*
Ms. Clark was referred to the Sona Access Program from a local skilled nursing facility.
She had recently undergone a surgical operation and also suffered from multiple complex diagnoses.
She lives alone, but has a daughter that lives out of state. Her daughter would come into town to visit her Mom, pick up medications from her former pharmacy, and spend her time filling a monthly pill box.
The Sona Access team partnered with all of Ms. Clark’s providers and reconciled her complex medication list.
Her specialized providers were unaware of drastic medication changes during her hospital and skilled nursing facility admissions. The Access team was able to serve as a liaison between the providers so this patient had exactly what she needed.
Now, when Ms. Clark’s daughter comes into town for a visit, she spends time with her Mom instead of filling her pill box.
