The Sona Access Program is a service that provides personalized assistance for managing difficult medication regimens.
The goal of the program is to help patients take the correct medications at the right time to improve their health.

Ms. Smith had been living in a group home for over a decade with 24-hour care, including someone managing all of her medications.
Several months ago, she called our team and said she was transitioning to her own housing in independent living. She was aware of the barriers she would face and knew she needed assistance with medication management.
We partnered with the facility she had been living in to coordinate care for her return home. The Sona Access team reviewed medication lists from multiple providers and reconciled the lists to ensure she would have everything she needed.
Ms. Smith was able to move to her home without any issues. She now trusts the Sona Access team with her medications and tells us how very thankful she is to be home and independent!
Ms. Johnson was referred to us by a community partner due to her very complex medication regimen.
Ms. Johnson lives alone and has a very difficult time finding transportation to the pharmacy. With her complex list of medications, she was having to make multiple trips to her pharmacy every week. This led to a lot of confusion and frustration.
The Sona Access team obtained medication lists from all of her providers and transfers from her former pharmacy. We were able to package her medication and start delivering her medications for free.
Ms. Johnson no longer has to worry about filling her pill box or finding transportation to the pharmacy.
She recently told our staff that the Sona Access program has been such a help to her and she was extremely happy she decided to enroll!

Prior to using Sona, Jill and her husband Murphy were having to go to a pharmacy and wait in line for sometimes hours at a time to refill their medications. They would do this multiple times a month and often multiple times a week.
Since they’ve transferred to Sona and trusted the team to manage their medications for them, their routine has become a lot easier.
They no longer have to worry about when each medication will be due or standing in line at a pharmacy. The Sona team calls them monthly, reviews their medications, and then delivers their prescriptions straight to their door free of charge.
Jill was so pleased with the service! After her first experience of using Sona and receiving all her medications in one delivery Jill said: “You are so awesome! You have no idea how much of a relief it is to not have to worry about our meds each month!! Thank you so so much!”
