“3 years ago, my mom broke her back several times in one year because of osteoporosis. She was in and out of the hospital and rehab facilities for a year. Once it was time for her to come home, I was looking for something that would help her manage her meds so I wouldn’t have to worry about her getting into them, taking the correct ones, or anything like that. That’s when we were introduced to the Access program and it was exactly what she needed.”
Caring for our patients’ needs and supporting caregivers is our number one priority. The Access team sees themselves as an extension of your family. We call every month to check on the patient, but we also like to call just to see how our caregivers are doing with the added stress that comes with being a caregiver.
The Access program focuses on removing the burden of organization and timing multiple medications, as well as coordinating with doctors, updating medication lists, and managing the patients’ medication adherence. One of the biggest fears caregivers have is accidentally missing a dose or confusing one medication with another. Especially when caring for someone dealing with a complex health concern, medication adherence is critical.
We off a variety of packaging, but our most preferred is our 7-day pill packs that have all medication packaged together into one bubble based on the time of day the patient should take every medication. They also show a photo of the pill or pills for that time slot so there is no confusion about which medication it is. Our pill packs run Sunday-Saturday with time slots for Morning, Noon, Evening, and Bedtime medications. We know filling a pillbox can be time-consuming and sometimes confusing, let us take that stress off of you so the time spent filling a pillbox can become time spent with your loved one.
We also work very closely with our patient’s physician to make sure each medication being packaged is accurate. We’re able to make changes to pill packs as medications change for our patients, and oftentimes, we help keep physicians aware of what other physicians are prescribing for the same patient.
This removes a huge burden for caregivers who feel like they have to keep track of all of the medications being prescribed from a large number of physicians.
We have technicians as well as pharmacists available to virtually meet with you to further assist in the training/education of our services as well as several other things. Sometimes it takes a team to provide the support our patients need to keep them safe and at home as long as possible.
