This personalized session includes:

Health screenings like blood pressure and fingerstick blood sugar tests. You may also add on Afinion2 A1C (blood sugar) and Colestech lipid (cholesterol) panel tests for an additional fee. These results will be reviewed with you and communicated to your doctor.

Medication review with the Clinical Pharmacist to learn more about your medications and ensure you are taking the medications best suited to your needs. Any opportunity for medication optimization can be communicated to your doctor.

Personalized conversation about what a healthy lifestyle – including diet, exercise and more – looks like for you. We will also provide take-home materials to keep you on track after your appointment.
